I am a Physical Therapist Primary Care Clinician working for Sutter Care at Home , Novato, Marin County. I primarily treat geriatric and adult population. Responsibility includes Start of care – Admitting patients to agency, completing assessment, OASIS scoring for Medicare, complete physical therapy evaluation, developing and executing Plan of Care, supervising PTA’s and guiding them to provide appropriate care to patients, case managing and supervising complete care of patient with keeping other disciplines – (Nurses, Occupational therapist, Home Health aide , Medical Social worker) in loop and engaged in care and meeting patients needs. Heavy case load management 25 to 30 patient at a time. Also keeping patients doctor updated about patients current health status and making sure patient needs are met and they are safe at home. Ordering Durable medical equipments for patients in need and maximizing there safety in home. I played a significant role through out the Covid period by seeing the Covid positive cases from beginning of March 2020 and continuing since. No vaccines were available in beginning and virus was very new to society. I treated Covid patients with special physical therapy skills and got them to full recovery and back to normal life.
